What should the quotation say about design changes after approval?
State that the price is based on the approved wireframes and that new or changed screens are quoted as additions. Without that sentence every late idea turns into a negotiation about whether it was included.
Should maintenance after launch be in this quotation?
Quote it as an option on its own line or in a separate quotation, for example a monthly fee for operating-system updates and fixes. Mixing it into the build price hides what the launch itself costs.
How do I quote when the client has not chosen between iOS and Android?
Quote both platforms on separate lines so the client can approve one or both. If you build with a cross-platform framework, say so, because it changes what a price per platform means.
How is the sales tax worked out on this quotation?
